What the decoder does
It looks for common language patterns such as missing information, eligibility, exclusions, timing, verification and evidence sufficiency. These are organizational signals, not legal conclusions. The actual meaning comes from the full decision, contract, policy or governing rules.
Do not paste the whole private file
Use only the wording needed to understand the reason. Remove names, addresses, account numbers, medical details and other unnecessary sensitive information. Keep the complete original document in your own secure records.

Quality check before you act
