Denials
Claim denial decoder
Pull the stated reason, missing items and questions to verify out of a denial notice.
Open →Practical paperwork organization
PaperworkOS helps you extract the important facts from denials, forms, visa paperwork, refund disputes and complaint records. Build a timeline, organize evidence, draft a factual response and keep the official source in view.
PaperworkOS is an independent information and organization resource. It is not a government agency, law firm, migration agent, insurer, tax adviser, financial adviser or medical provider. Tools and guides help you organize facts, documents and questions; they do not determine eligibility, rights, deadlines or outcomes. Always confirm current requirements with the relevant official source.

Open →A reusable workflow
Keep the original document intact. Record who sent it, when it was issued, when you received it, any reference number and every date stated in the document. If a phone call matters, write a dated note after the call.
Separate the sender's actual words from your interpretation. Quote the reason or instruction accurately in your private notes, then list what is unclear. This prevents a response from drifting away from the issue that must be addressed.
Match each important fact to evidence: a receipt, policy wording, application record, email, screenshot, report, identity document or other source. Mark missing items instead of pretending the file is complete.
Check the current official process, form, portal, fee and deadline. Then send a focused response, retain proof of submission and schedule a follow-up date.
